All of this was due to a reconnect bug and all keypads were replaced as soon as it happened.

After looking into the report it does show he did reconnect earlier on. Reconnecting or re-pasting dupe tends to cause issues with keypads. Please remember to change or check your keypads after reconnecting / pasting dupe. This is a warning.
